Aging can be isolating and challenging, but Elevate is here to help all adults age well, regardless of background, ability, or circumstance. Our program addresses the unique needs of adults in our area, promoting intentional action to live well.
Aging well isn’t just about happiness—it’s also cost-effective. Staying healthy and in one’s own home reduces expenses for individuals, families, and the healthcare system.
Through partnerships with local organizations, Elevate connects residents to resources and opportunities to improve physical, mental, and social health. From eating well and staying active to understanding Medicare, we help optimize health for all.

Meet the Team
Mindi Larson
Program Manager
As our Program Manager, Mindi’s primary duties are to continue to identify needs within our communities, continue to create programming that matches those needs, and work to find ways to keep Elevate as a permanent fixture once our pilot phase ends. Â
When she’s not being a Nosey Neighbor, you can find Mindi on a frozen lake, tip-up in hand, fishing for her beloved Northern Pike. Or camping with her husband (Scott), daughter (Jovie), and two pups (Norman and Leo). Â
Katie Murdock
Community Coordinator
As our Community Coordinator, Katie’s role is primarily out and about in the community. You can find her planning and attending events with local businesses and organizations, coordinating volunteer drivers, working on referrals from the clinic, and so much more! Â
She and her husband, Nick, have 3 children, a mischievous Small Münsterländer, and enjoy more hobbies than they have time for!Â
Emily Dreyer
Communications Coordinator
As our Communications Coordinator, Emily is the creative force behind the scenes, making sure social media shines, the website runs smoothly, and the beloved monthly newsletters get printed in time. Â
When Emily steps away from her screen, she and her husband, Michael, are weekend warriors, filling their days with travel adventures, outdoor fun, and as many new experiences as they can pack in.Â
Jean Tobkin
RN Health Coach
As our RN Health Coach, Jean is on the frontlines connecting with patients and ensuring all of their healthcare questions are answered. Â
When she isn’t driving patients to appointments on her days off, you can find Jean spending time with her husband (Nate) and daughter (Ava). Â
Amanda Kaldor
Connections Coordinator
As our Connections Coordinator, Amanda oversees our daily lunch and programming at the Perham Area Community Center. With a background as a Registered Nurse and Health Coach, Amanda has a passion for staying active and helping adults discover how they can enjoy healthy living in a maintainable way. Â
She is a proud wife and mom of three, and when she’s not working, you can find her cheering on her kids at their activities—Go Yellowjackets!Â
The Story of Elevate
What began as a community-driven idea to better connect resources for older adults has grown into a thriving program dedicated to helping people age well. Elevate’s journey reflects the power of collaboration, innovation, and a shared commitment to making aging a positive experience for everyone. From its humble beginnings with Post-it notes on a wall to becoming a trusted resource in the community, Elevate’s history is a testament to what’s possible when people come together to address meaningful challenges.
2016: Silos to Circles
In 2016, a grant from LeadingAge Minnesota Foundation provided seed money to address how to help people stay home longer and safer amidst the “silver wave” of aging. Community conversations brought together stakeholders, including individuals, hospitals, counties, churches, cities, and service providers, to brainstorm better ways to connect resources. During listening sessions, attendees filled an entire wall with Post-it notes listing local senior services, underscoring the need for coordination rather than new services. This effort led to the creation of The Connection, a neutral organization designed to act as a local navigator and build trust within the community.Â
Late 2018: The Connection Takes Off
In late 2018, The Connection hired its first navigator and introduced a contact number to provide support. However, gaining visibility was a challenge without a strong platform to promote the service, and just as momentum was building, the COVID-19 pandemic created significant disruptions to outreach efforts.Â
2020-2021: Building Momentum
By summer 2020, discussions began around applying for the Connected Communities Grant to support a broader vision for the program. In January 2021, The Connection was awarded $800,000 through funding from LeadingAge Minnesota Foundation and UCare. This milestone brought Sue Huebsch into the fold, whose leadership and community connections were instrumental. After a year of research and planning, Elevate was born, grounded in four key pillars and a mission to elevate and connect existing services rather than compete with them.Â
2022: Elevate Launches
Late 2021 saw the finalization of Elevate’s branding, including its messaging, website, and social media presence. These efforts culminated in the official public launch of Elevate in early 2022, marking the beginning of a new chapter in community connection and support.Â
2023-2024: Expanding Engagement
The year 2023 marked significant growth and innovation for Elevate. Programs like the Exploratories and Nosey Neighbor tours launched early in the year, offering new ways to engage the community. Summer 2023 saw the debut of the Elevating Voices Memory Loss Chorus, bringing music and connection to individuals with memory loss and their families. Additionally, Elevate continued to expand its educational and wellness opportunities, setting the stage for even greater impact. This momentum carried into March 2024, with the introduction of the Connections daily lunch program, further enhancing our mission to foster health, connection, and joy across the community.
