Welcome to the Healthspan Challenge!

The Healthspan Challenge is a 6-week community wellness challenge focused on helping you build healthier habits through small, weekly actions. Each week, you’ll complete simple challenges designed to support your physical, mental, or social well-being.

Participants turn in a completed slip each week to be eligible for weekly prizes, and those who complete all six weeks by Tuesday, September 30 will be entered to win the grand prize of $250 in Perham Bucks.

It’s all about progress, not perfection; proving that it’s never too late to make changes that help you feel your best.

Who can all join the Healthspan Challenge?

Any and all ages are encouraged to register and participate in the Healthspan Challenge!

Where can I find a Healthspan Challenge Tracking Sheet?

Pick up a Healthspan Challenge tracking sheet at any of our info stands (Perham Area Community Center, Perham Living, Perham Public Library, and New York Mills Public Library) or print one yourself here!

Can I register in person?

Yes! Join us at our Healthspan Challenge Kickoff on Tuesday, August 13 from 8-11am at Perham Area Community Center to register in person. We’ll also leave a registration sheet at the PACC for signups throughout the Challenge. Ask the front desk or give us a call at 218-347-1974.

The Challenge has already started. Can I still join?

YES! You can join the Challenge and complete it any time before Monday, September 30. The sooner you sign up, the more chances you have to win prizes!

Where do I submit my completed Healthspan Challenge?

You can turn in your completed Challenge in one of three ways:

  • Give it to an Elevate team member at any Elevate event
  • Drop it off in the Healthspan Challenge basket at Perham Area Community Center front desk
  • Email a picture of it to connect@elevateotc.org

ALL Challenges must be submitted by Monday, September 30!

Do I have to complete the activities in a specific order?

Nope! You can complete the weekly activities in any order that works for you—just be sure to finish at least five by the end of the week.

Don’t forget: To be eligible for that week’s prize, your completed slip must be turned in by the due date listed. No early birds, no late entries!

How do I track my progress?

Use your weekly challenge sheet to check off activities as you complete them. Each week has its own slip where you can track your completion of at least five challenges. Then turn it in by the due date to stay in the running for prizes. You can pick up your tracker sheets starting August 15 at our Elevate info stands!

Can I participate if I have physical limitations?

Absolutely! The Challenge includes a variety of activities that cater to different abilities and interests. Choose (or modify) the activities that best suit your needs and preferences.

Are there any costs associated with participating in the Challenge?

Participation in the Healthspan Challenge is free, and there are many free (or low cost) opportunities available throughout the Challenge.

How are winners selected for prizes?

Each week, everyone who turns in a completed challenge slip by the due date is entered into a drawing. We randomly select winners from that week’s entries. The same goes for the grand prize: complete all 6 weeks and turn in all your slips by September 30 to be entered to win $250 in Perham Bucks!

Who can I contact if I have more questions?

If you have any additional questions, please contact us at connect@elevateotc.org or call 218-347-1974.

Why should I participate?

Because your health is worth it, and small steps can lead to big changes. The Healthspan Challenge is a fun, low-pressure way to build better habits, feel more energized, and connect with your community. Plus, there are prizes!
